This implies that there is a possibility that you pass the exams but you cannot join the lab you choose for some reason (e.g., the lab capacity exceeded). Omitting choosing the second/third labs will lead to your being dispatched to a random lab, which we believe is not what you want normally.

I've got a letter from the only lab I choose, and the professor of the lab says they accept me. Why on earth do I still need to indicate laboratories of my second or third choices?

You can convert your responses in Microsoft Forms into a PDF using the print function on your PC or Mac.
On smartphones or tablets, this can also be done using the print or share function, which often allows you to save the page as a PDF.
If you have difficulty creating a PDF, screenshots of your completed responses are also acceptable, but please make sure to combine them into a single file before uploading.
I could not find how to upload the Inquiry Form. How can I upload it?
On the document upload screen, select both the file to be uploaded and the item to which it applies from the list. In the list, there is a "Others" item. You must upload the Inquiry Form.
